BridgeCare Foundation - About Us: Building bridges, Restoring hope, Creating stability

Our Mission

To provide compassionate, non-clinical support to individuals transitioning from hospital discharge to community life, addressing the non-medical barriers that prevent safe, stable recovery.

Our Vision

A future where no one leaves the hospital without the support they need to safely transition back into their community, with dignity, hope, and a clear path forward.

Antonia Giordani

Founder & Executive Director

With a background in nursing and a deep commitment to social equity, Antonia founded BridgeCare Foundation after witnessing firsthand the gaps that exist between medical discharge and true recovery.

She saw patients leave hospitals with nowhere to go, no transportation, no food, and no community support, medically cleared but far from stable. BCF was born from her resolve that this does not have to be the norm.

Antonia leads BCF with a hands-on, person-first approach, building partnerships, inspiring volunteers, and ensuring every person who reaches out receives the dignity and care they deserve.

"No one should have to face their recovery alone. That is why we exist, to bridge the gap between a hospital bed and a stable life."
