Founder & Executive Director
With a background in nursing and a deep commitment to social equity, Antonia founded BridgeCare Foundation after witnessing firsthand the gaps that exist between medical discharge and true recovery.
She saw patients leave hospitals with nowhere to go, no transportation, no food, and no community support, medically cleared but far from stable. BCF was born from her resolve that this does not have to be the norm.
Antonia leads BCF with a hands-on, person-first approach, building partnerships, inspiring volunteers, and ensuring every person who reaches out receives the dignity and care they deserve.
"No one should have to face their recovery alone. That is why we exist, to bridge the gap between a hospital bed and a stable life."
, Antonia Giordani
